Your first conversation
Whether you are a family member, a case manager or a clinician, the first call is simple and there is no obligation.
Tell us who you are and what you need. You do not need paperwork to ask a question.
We walk through how home health works, what the physician needs to order and what coverage may apply.
If home health fits, we coordinate with the physician. If it does not, we tell you plainly and point you to the right kind of help.
Your safe harbor for care at home. Care begins with a referral from a physician, hospital, clinic or family. Our intake team verifies coverage, confirms the physician's orders and schedules a start-of-care visit with a registered nurse or therapist. From there, the plan of care guides every visit, and the physician's office hears from us whenever something changes.
We serve adults recovering from surgery, hospital stays and chronic illness across North Texas, from our office in Frisco. Care is ordered by the patient's physician and delivered at home by registered nurses, therapists, medical social workers and supervised home health aides. Free language assistance is part of how we work from the first call.
