This notice applies to this website only. It is operated by Gracious Care Health Services, Inc., an OceanCare Health company ("OceanCare Health"). Patient health information is governed by our Notice of Privacy Practices.
Information you submit
Our forms collect contact details, the topic of your request and, for job applications, your resume. We use this information only to respond to you or evaluate your application. Form submissions are delivered to our team by email and to our customer relationship management system, which receives contact details only. Resumes are delivered by email only.
Please do not send health information
Our website forms are not intended for protected health information. Do not submit patient names, dates of birth, diagnoses or other medical details. When care is being arranged, our team collects that information through secure channels.
Analytics and cookies
We may use analytics tools, such as Google Analytics through Google Tag Manager, to understand which pages are visited and which buttons are used. Analytics never receive the contents of form fields, quick-check answers or health information. Our analytics events record only that an action occurred, such as a form submission or a phone link click.
No sale of personal data
We do not sell personal data and do not use it for targeted advertising based on health information. We share it only with service providers who help us operate the website and respond to requests, under confidentiality obligations, or when required by law.
Your Texas privacy rights
Depending on how the Texas Data Privacy and Security Act applies, you may have the right to access, correct, delete or obtain a copy of personal data we hold about you, and to opt out of certain processing. Security and retention
Our site is served over HTTPS. We keep form submissions only as long as needed to respond and to meet legal record-keeping obligations.
